Leomed combines the gastroprokinetic action of Levosulpiride with the acid-neutralizing benefit of Magnesium Hydroxide, offering a dual-action therapy for functional dyspepsia, gastroesophageal reflux disease (GERD), and acid-related upper GI symptoms.

This combination addresses both motility dysfunction and gastric acid excess, making it particularly suitable for patients with bloating, early satiety, nausea, and heartburn of functional origin.

Usage of Leomed

Indicated for the management of:

  • Functional dyspepsia

  • Gastroesophageal reflux disease (GERD)

  • Chronic gastritis with reflux symptoms

  • Bloating, epigastric fullness, and nausea

  • Delayed gastric emptying

  • Non-ulcer dyspepsia

  • As adjuvant in irritable bowel syndrome (IBS-D) with upper GI symptoms


Mechanism of Action

🧠 Levosulpiride (75 mg)

  • A substituted benzamide and selective dopamine D2 antagonist

  • Enhances gastric and intestinal motility by promoting acetylcholine release in the enteric nervous system

  • Improves symptoms of gastroparesis, bloating, and epigastric distress

  • Also possesses mild anxiolytic and antidepressant effects via central D2 receptor blockade

🧪 Magnesium Hydroxide (as trihydrate, 40 mg)

  • Acts as a rapid-acting antacid

  • Neutralizes excess gastric acid, offering quick relief from heartburn and acidity

  • Also has a mild laxative effect which helps counteract gastric stasis


Common Side Effects

  • Abdominal cramps or bloating

  • Diarrhea (due to magnesium component)

  • Mild sedation or fatigue (rare, due to levosulpiride)

  • Galactorrhea or menstrual changes with prolonged use (levosulpiride)

  • Dry mouth or metallic taste


Precautions

  • Contraindicated in patients with GI hemorrhage, perforation, or obstruction

  • Use cautiously in renal impairment (due to magnesium retention risk)

  • Avoid in pregnant or lactating women unless absolutely necessary

  • Long-term use may lead to hypermagnesemia or extrapyramidal symptoms


Drug Interactions

  • Levosulpiride:

    • May enhance the effects of CNS depressants

    • Use cautiously with antipsychotics (dopaminergic interactions)

  • Magnesium Hydroxide:

    • May reduce absorption of tetracyclines, fluoroquinolones, iron, and digoxin

    • Give at least 2 hours gap with other oral medications


Dosage and Administration

  • Usual adult dose: One tablet twice daily, 30 minutes before meals

  • Duration: Based on indication, typically 2–4 weeks for functional dyspepsia

  • Do not crush or chew; swallow with water


Storage

  • Store below 25°C in a dry place

  • Protect from moisture and light

  • Keep out of reach of children


Patient Counseling Points

  • Take on an empty stomach (preferably before meals)

  • May take a few days to show full benefit in functional GI disorders

  • Avoid taking antacids or dairy within 1 hour of this medicine

  • Report any signs of menstrual changes, breast tenderness, or excessive fatigue if on long-term therapy

  • Maintain hydration if mild diarrhea occurs
