Benign Prostatic Hyperplasia (BPH)

May be used as monotherapy or in combination with 5-alpha reductase inhibitors
Common Side Effects

Retrograde ejaculation (common but reversible)

Dizziness or postural hypotension (less frequent due to uroselectivity)

Nasal congestion

Diarrhea or mild GI upset

Headache, fatigue (rare)
Retrograde ejaculation is dose-dependent and usually resolves upon discontinuation.
Precautions

Postural hypotension risk—use caution in patients with low baseline BP or on antihypertensives

Hepatic impairment: Use with caution

Severe renal impairment (CrCl <30 mL/min): Not recommended
-
Not for use in females or children
-
Cataract surgery: Risk of Intraoperative Floppy Iris Syndrome (IFIS)—inform ophthalmologist before surgery

Avoid alcohol while on treatment
Drug Interactions

CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin): May increase silodosin levels—avoid co-administration

Antihypertensives: Additive hypotensive effect possible

Phosphodiesterase-5 inhibitors (e.g., sildenafil, tadalafil): Use with caution—monitor for BP drop
-
Alpha-blockers: Do not combine with other α₁-blockers
Dosage and Administration

Silodosin 8 mg once daily, preferably with food to improve bioavailability and reduce adverse effects

Dose adjustment in moderate renal impairment (CrCl 30–50 mL/min): 4 mg once daily

Swallow whole; do not crush or chew
Storage

Store in a cool, dry place below 25°C

Patient Counselling Tips

Take the capsule with meals for best effect and tolerability
-
May cause dizziness—rise slowly from sitting or lying position
-
Inform about the possibility of retrograde ejaculation—this is harmless and reversible
-
Avoid driving or operating machinery if dizziness occurs
-
Inform eye surgeon before cataract surgery about Silodosin use
-
Do not discontinue medication abruptly without consulting your doctor
